Additional Qualifier Information for Non-Citizens and players who have belonged to other federations

To enter a qualifier tournament which may lead to being a member of a team respresenting the FPUSA in a wolrd championship tournament, non-citizens and players who have belonged to other FIPJP Federations must complete this form as a part of their registration for that tournament.
